Strategic Plan & Data Dashboard

The Strategic Plan and Data Dashboard, created by the Network, provide a comprehensive view of our objectives and outcomes, enabling stakeholders to stay informed, make strategic decisions and drive meaningful progress. Explore this section to understand our plan and monitor our impact.

Explanation of the Network Development Grant

The Food Access in Health Care (FAHC) Network project consists of four large goals:

1) Improving food insecurity screening and referral infrastructure

2) Improving nutrition insecurity screening and referral infrastructure

3) Improving medically tailored food systems, which includes everything from giving individuals more access to fruits and vegetables to providing medically tailored meals for individuals with multiple chronic conditions, and everything in between, and moving all of those systems forward

4) Knowledge dissemination, including training for medical staff, food system partners, and others, and knowledge dissemination to the public about the food programming in Vermont.
